Records Request
Requesting EMS Patient Care Records

Patient care records may be requested by:

  • The patient directly.
  • A legal representative of the patient with appropriate documentation such as a signed HIPAA authorization, Personal Representation, Personal Attorney/Healthcare Surrogacy, etc.
  • A requestor authorized by law through subpoena, court order, or search warrant.

If you meet one of these criteria, please submit your request by emailing: emsrecords@greenvillecounty.org.

There is a $25 fee per record.

All requests must include:

  • The patient’s full name, date of birth, and date of service.
  • Proof of identity or legal authority to obtain the records.
  • Contact information for follow-up.
  • Records that you are requesting i.e. patient care report, 911 audio, CAD report, etc.
Requests Under the South Carolina Freedom of Information Act (FOIA)

FOIA requests for EMS incident records will be redacted in accordance with state and federal laws to protect Personal Health Information (PHI) as required by HIPAA and other applicable regulations.
